6. Outcomes of the practice

The output of the professional practice is the Midwife’s Logbook. The primary purpose of the Midwife’s Logbook is to record professional practice across various clinical settings and to serve as a basis for the assessment of students during these placements.

The logbook contains general information regarding the practice, details on the progression of practical training, learning objectives, and records of Health and Safety (OSH) and Fire Safety (FS) inductions—conducted both at the commencement of the academic year and at individual workplaces. It is used to document specific clinical procedures, including the examination of women during pregnancy, labour, and the postnatal period, as well as neonatal examinations.

The logbook must be presented for final evaluation at the end of each semester, as well as during the final practical examination and the State Final Examination at The College of Polytechnics Jihlava.
